Mind Body Works Telehealth LLC (“MBW Telehealth,” “we,” “us”) operates www.mindbodyworks.us and the MBW Patient Portal. This policy explains what information we collect through the website, the patient portal, and text messaging, and how we use and protect it.
1. Health information and HIPAA
As a health care provider, MBW Telehealth is subject to the Health Insurance Portability and Accountability Act (HIPAA). Protected health information in your medical record is used and disclosed only as described in our Notice of Privacy Practices, which is provided in the patient portal at registration.
2. Information we collect
- Registration information: name, date of birth, address, email address, mobile phone number, and insurance information.
- Health information you provide in questionnaires, visits, messages, and lab results.
- Payment information, collected and stored by our payment processor. We do not store full card numbers.
- Technical information needed to keep you signed in to the portal securely. We do not use advertising or tracking cookies.
3. How we use information
We use your information to provide care, schedule visits, verify insurance, process payments, send appointment and questionnaire reminders, respond to your messages, and meet legal and professional obligations. We do not sell personal information.
4. Service providers
We use service providers for secure hosting, telehealth video, payment processing, laboratory testing, and message delivery. Providers that handle protected health information do so under a Business Associate Agreement. Our payment processor receives only the information needed to process a payment, never diagnoses or reasons for visits.
5. Mobile information and text messaging
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

6. Security
The website and portal use encrypted connections (SSL/TLS), access controls, and HIPAA-compliant hosting under a Business Associate Agreement.
7. Your choices and rights
You may request access to or a copy of your records, request corrections, update your contact information in the portal, and stop text messages at any time. Your rights regarding health information are described in our Notice of Privacy Practices.
8. Adults only
MBW Telehealth serves adults 18 years and older. We do not knowingly collect information from children.
9. Changes to this policy
We may update this policy. The effective date above shows when it was last revised.
