GET YOUR NEW YORK STATE MEDICAL MARIJUANA CARD IN THREE SIMPLE STEPS!!

Step 1:   Determine if you have a qualifying medical or mental health condition and also meet NY state residency requirements.

Step 2:   Obtain a certificate of eligibility from a NY authorized MMJ certifier.

Step 3:   Register with the NY Department of Health (DOH).

STEP 1 – Determine If You Qualify

You may qualify for a NY MMJ card if 1) you have a qualifying medical condition that is accompanied by one or more specific complicating conditions or if you have a qualifying mental health condition; AND 2) you meet the NY state residency requirements.

Qualifying medical or mental health conditions are listed below.

A.  QUALIFYING MEDICAL CONDITIONS

 Inflammatory Bowel Disease (IBD)

 Diabetes

 Fibromyalgia

 Neuropathy

 Multiple sclerosis (MS)

 Arthritis

 Glaucoma

 HIV/AIDS

 Amyotrophic lateral sclerosis/Lou Gehrig’s disease

 Parkinson’s disease

 Spinal cord damage with intractable spasticity

 Epilepsy

 Cachexia (wasting syndrome)

 Crohn’s disease

 Lupus

 Huntington’s disease

                                         AND

The Qualifying Condition is accompanied by ANY of the following complicating conditions:

 Cachexia or wasting syndrome

 Severe or chronic pain

 Severe nausea

 Seizures

 Severe or persistent muscle spasms

    B.  QUALIFYING MENTAL HEALTH CONDITIONS

 You may also qualify if you have EITHER of the following MENTAL HEALTH conditions:

 Opioid Use Disorder

 Post-traumatic Stress Disorder (PTSD)

    C.  OPIOID REPLACEMENT

      Medical Marijuana may be recommended to treat any condition for which an opioid could be prescribed.

    D.  QUALIFYING NEW YORK STATE RESIDENCY REQUIREMENTS

You must be a resident of New York State (NYS) OR receiving treatment in New York State for one of the above-listed conditions

STEP 2 -- Obtain a certificate of eligibility from an authorized NY MMJ evaluator.

STEP 3 -- Register with NY Department of Health’s online patient registration system

  

To Register for Your MMJ Card:

  Go to https://my.ny.gov/.  

If you don’t already have an account, create one by clicking on the “Don’t Have an Account?” button.

  Click the "Health Applications" icon, and then click the "Medical Marijuana Data Management System" link to register.

NOTE:  Patients will be required to provide proof of their identity and residency during the application process.

ALSO NOTE THAT: Pursuant to the Compassionate Care Act there is a $50 application fee; the Department is currently waiving the $50 fee for all patients and their designated caregivers.

  If you wish, you can designate up to two caregivers during the registration process. After your registration is processed,

the designated caregiver(s) must then register with the Department using the same online system described above.
